COMMUNITY WELLBEING

For the 2022-23 fiscal year, the community wellbeing department expanded mental health and social health programming to include health and system service navigation - including case management - to help residents access life stabilization supports, and to address the social determinants of health.

During this time, 76 residents in the community accessed service navigation with case management, many requiring long-term support due to complex mental and social health issues. By adopting a community health service integration approach and collaborating with service providers from the mental health and social services sectors to deliver group-based programming, we have significantly improved the client experience.

Our Seniors Social Program, which provides senior residents with group programming focused on mental and physical health, experienced a 225% increase in participation compared to the previous year, with new participants attending weekly programming and with over 90% retention rate.
